Blue Sky Community Services has partnered with The Mid North Coast Local Health District on the ‘Enhancing Inclusion in the Healthcare System’ project, funded by the Department of Social Services under an ‘Information, Linkages and Capacity Building’ grant.

The project aims to develop a best practice model for enhancing the inclusion of people with disability in the mainstream health system in NSW, by strengthening the skills of health staff in supporting people with disability and improving coordination between health and local NDIS providers.

It includes a training program for health care staff, based on best practice principles, to build their capacity to communicate with and provide high-quality care to people with disability.

It will develop systems which support efficient communication between health staff and local NDIS providers, to improve the overall hospital experience and health outcomes of people with disability.

The project is being delivered by two Disability Healthcare Specialists in the Mid North Coast Local Health District, to strengthen the delivery of inclusive and relevant health care to people with disability across five hospitals.
A regional consumer working group comprising people with disability is being consulted to provide additional lived experience and advice.

Hospital Care Resources

The Inclusive Hospital Care Resource provides prompts for discussion between people with disability in hospital and the Health staff supporting them. Working through this process as a team supports person-first inclusive care that meets the needs of the patient throughout the hospital admission and discharge processes. The front page of this resource serves as a cover sheet for information about the person that would be useful to share with Health staff.

The My Health Journey Passport provides a template for people with disability to record important information about themselves that is useful to share with the Health staff who support them during their hospital journey.

If you would like to make use of this passport please download it, record your information, print it and attach it to the back of the Inclusive Hospital Care Resource. It might be useful to keep your information in a plastic sleeve so it is protected and can be kept handy in preparation for going to hospital.
